If you are/were an Indiana citizens whose Private Information was allegedly disclosed to third parties through Reid Health’s use of the Meta pixel or any other website tracking, analytics and/or advertising technologies on its Websites, then you may be eligible to receive Settlement Benefits.

If you are a Settlement Class Member and wish to receive a payment, you must complete a Claim Form must be submitted online or postmarked on or before December 24, 2025.

Medical Shield Product: All Settlement Class Member will automatically receive a code to enroll in the Medical Shield product. The code is listed at the top of the Notice that Class Members received. Instructions for how to enroll in the Medical Shield product for one year for free will automatically be sent to you later in a separate communication.

Cash Payment: Each Class Members can claim a one-time $25.00 cash payment.

To start your online claim, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailing info@ReidHospitalPixelSettlement.com or (844) 496-1154. Please request your Unique ID & PIN by providing your full name and mailing address.
